Rapid Palatal Expander (RPE) Instructions

What is a Rapid Palatal Expander?

A Rapid Palatal Expander (RPE) is an orthodontic appliance that gently widens the upper jaw. This helps create space for teeth, improves the bite, and can support better breathing.

How the Expander Works

  • The expander is attached to your upper teeth.

  • A small key is used to turn the screw in the center of the appliance.

  • Each turn makes a tiny adjustment that slowly widens the upper jaw.

You may feel pressure, but you should not feel sharp pain.

How to Turn the Expander

Your orthodontist will tell you exactly how often, either once daily OR twice daily.

Steps:

  1. Wash your hands.

  2. Sit in good lighting with a mirror.

  3. Insert the key into the hole of the screw.

  4. Push the key straight back toward the throat until it stops.

  5. You will see a new hole appear at the front—this means the turn is complete.  If the new hole is not fully visible, then the previous turn was not pushed all the way back.

  6. Carefully remove the key to not accidentally undo your new turn.

  7. Do not turn the screw backward.

⚠️ Important: Only turn the expander as instructed. Do not skip or add extra turns.

What to Expect

  • Pressure or tightness in the roof of the mouth

  • Mild soreness in teeth or jaw

  • Temporary changes in speech

  • A space forming between the front teeth (this is normal and expected)

These symptoms usually improve within a few days.

 

Managing Discomfort

  • Mild discomfort is normal, especially the first few days.

  • Over-the-counter pain relievers (like acetaminophen or ibuprofen) may be used if needed.

  • Soft foods can help during the adjustment period.

Call us if pain is severe or does not improve.

 

Eating With an Expander

Recommended foods:

  • Yogurt, smoothies, eggs

  • Pasta, rice, mashed potatoes

  • Soft fruits and cooked vegetables

Avoid:

  • Sticky foods (gum, caramel, taffy)

  • Hard foods (ice, nuts, hard candy)

  • Chewy foods (bagels, tough meats)

 

Cleaning & Oral Hygiene

  • Brush teeth and the expander after every meal.

  • Use a toothbrush to clean around the appliance carefully.

  • A water flosser can help clean hard-to-reach areas.

  • Rinse with water after eating if brushing isn’t possible.

Good hygiene prevents cavities and gum irritation.

 

Do’s and Don’ts

DO

  • Follow turning instructions exactly

  • Call if something feels loose or broken; if the expander comes loose - please bring to the next appointment (if it is lost, there will be a replacement cost)

DON’T

  • Skip turns unless instructed

  • Play with or pick at the expander

  • Try to adjust it without the key

 

When to Call the Office

Contact us if:

  • The expander becomes loose or breaks

  • You cannot complete a turn

  • Pain is severe or unusual

  • You lose the key

How Long Will I Wear the Expander?

  • Active expansion usually lasts 2–4 weeks, depending on what your orthodontist recommends.

  • The expander stays in place for several months (usually about 6 months) to allow the bone to stabilize
